Dr. Kenzou Tenma had a great future ahead of him. He was an elite neurosurgeon and was engaged to the hospital director's daughter. But one night, a small event changed his life. The hospital director called him and asked him to switch patients and perform life-saving brain surgery on a famous performer instead of the original patient. His fellow doctors, fiancée, and the hospital director praised him for his accomplishment. But his conscience was disturbed, since the switch had lead to a poor immigrant worker's death.

So, when a similar situation arose, Dr. Tenma chose to perform surgery on the young boy Johan Liebert instead of the town's mayor. As a result, he lost his social standing. However, with the mysterious death of the director and two other doctors, Dr. Tenma's position was restored. Although he was released with no evidence to convict him, nine years later his past came back to haunt him. He had to embark on a quest to make amends for the havoc spread by the one he saved.
